What to Bring to the Louisiana OMV for Your Learner's Permit

informational·how to guide·

Where this document list comes from

This list comes from the Louisiana Office of Motor Vehicles (OMV) New Licenses page, OMV policy 6.00 on identification and the school attendance law, R.S. 32:431.1, as of 2026. Other details come from the OMV Permits, REAL ID and appointment pages, OMV policies 3.04 (residency), 6.01 (driver education) and 8.00 (minors), R.S. 32:409.1 and the Louisiana Class D & E Driver's Guide. Confirm the rules with the Louisiana Office of Motor Vehicles (expresslane.org) before you drive.

School attendance (ages 15 to 17)A signed statement of school attendance, less than 90 days old.
Driver education proof (learner's permit)The sealed envelope from your driving school, unopened.

Two OMV visits: the TIP first, then the learner's permit

You apply to the OMV for a Class E Temporary Instructional Permit (TIP) before the knowledge test, the on-road lessons and the road skills test. So you go to the OMV two times. At the first visit, you get the TIP with the documents in this guide. After the driver education course, you go back and give the OMV your TIP with proof that you finished the course, to get the learner's permit. There is no extra cost to change the TIP to a learner's permit.

Primary and secondary ID documents for a first-time applicant

A first-time applicant brings one primary and two secondary documents, or two primary documents. Primary documents include an original U.S. birth certificate (or a certified copy), a U.S. passport or passport card and a current U.S. military ID card. If you are 17 or under and use a passport, also bring a certified birth certificate or custody or guardianship papers. If you already have a Louisiana ID card with a clear photo and your Social Security number is on file, you need only one more document. The OMV TIP page lists a shorter set, so bring the full set to be safe.

Proofs of address that the OMV accepts

  • A utility bill for services at your home, such as water, electric, cable, internet or phone.
  • A bank or credit union statement.
  • A school record, transcript or report card that shows your name and Louisiana address.
  • The Louisiana license, permit or ID card of a parent or guardian who lives with you, if that person comes to the OMV and shows it in person.
  • A current rental agreement that shows the dates you live there. The day of your OMV visit must fall inside those dates.

If you are under 18: who signs and what to bring

Your parents are not separated or divorced
A parent signs the application. You and the parent both show ID.
Your parents are divorced or separated and one parent has legal custody
The parent with legal custody signs. Bring the custody documents.
A court awarded joint custody
Only the domiciliary parent may sign. Bring the custody documents.
A legal guardian brings you
The guardian signs and shows ID, and also shows legal papers that prove the guardianship.

School attendance proof for applicants aged 15 to 17

Under R.S. 32:431.1, the OMV cannot issue a learner's permit or license to a minor (an unemancipated person aged 15, 16 or 17) unless proof of school attendance is shown when the minor applies. The law names both the TIP and the learner's permit. Ask your school principal for the signed attendance statement. In a home school program, your parent gives it. The statement is valid for 90 days from the date it is issued, so get it close to your OMV visit.

The sealed driver education envelope

Every first-time applicant must show proof of driver education. Your driving school puts your completed OMV knowledge test and your Certificate of Completion in a sealed envelope. Bring the envelope to the OMV unopened: only OMV staff may open it, not you or your parent or guardian. After the OMV issues the learner's permit, the rules for driving on a Louisiana learner's permit apply.

How to prepare for your OMV visit

  1. 1
    Check your record for flags

Before you book, check your driver's license status. If your record has flags, you must clear them before the OMV can give you any service.

  2. 2
    Book an appointment

Book a visit at an OMV customer service center.

  3. 3
    Gather your documents

Use the checklist above. Bring originals or certified copies from the issuer.

  4. 4
Bring your parent or guardian if you are under 18

That person signs the application and shows ID. Bring custody or guardianship papers if they apply to you.

REAL ID: two proofs of address instead of one

Louisiana issues both REAL ID compliant and standard cards, so you can choose, and both cost the same. A first REAL ID needs proof of identity, lawful status, date of birth and Social Security number, and two proofs of address. The two documents must show your name and Louisiana street address and come from separate sources, such as a utility bill and a bank statement. Since May 7, 2025, you need a REAL ID compliant card or another accepted ID, such as a passport, to board a commercial flight or enter federal facilities.

Key takeaways

  • Bring one primary and two secondary identity documents, or two primary documents, as originals or certified copies.
  • Add Social Security proof and at least one original proof of your Louisiana address. A REAL ID needs two proofs of address from separate sources.
  • Under 18, your custodial parent or legal guardian signs and shows ID. With joint custody, only the domiciliary parent signs.
  • At 15 to 17, bring a signed school attendance statement that is less than 90 days old. Keep the driver education envelope sealed.

Louisiana OMV permit documents: common questions

Can I give my Social Security number verbally at the OMV?

Yes, the OMV pages say you may give the number verbally. But under OMV policy 6.00, a number given verbally does not count as one of your secondary identification documents. So if you can, bring your Social Security card, a W-2 form or a printout from the Social Security Administration.

Can I use my school ID at the OMV?

Yes, as a secondary document. A Louisiana middle or high school photo ID counts. So do school records or two report cards from separate years, a health insurance card and a Social Security card. You still need a primary document, such as a birth certificate. The names on your primary and secondary documents must match.

What if my parent cannot come to the OMV with me?

If your parent cannot come in person, get the application form (DPSMV2003) at an OMV office and mail it to your parent. Your parent signs it, and the signature must be notarized. Then you bring the notarized application to the OMV with your identification documents.

Can I bring photocopies of my documents?

No. The OMV does not accept photocopies, even notarized ones. Bring each document as an original or as a certified copy from the office that issued it. If you have only a photocopy of your birth certificate, get a certified copy before you book your visit.

What if I lose my driver education certificate?

Ask your driving school or course provider for a duplicate of the original. The OMV accepts the duplicate when the owner, manager or director of the school signs it as a duplicate. Check the date too: the certificate expires five years after the course completion date.
